[Gmsh] problem with 2D mesh

I am using latest release (4.0.1, windows 64), and I am still having some problems with 2D meshes (see attached picture); for some surfaces the mesh degenerates, no matter which meshing algorithm I choose. The only way to get rid of this is to set Mesh.CharacteristicLengthExtendFromBoundary=1, which in general I would like to avoid. Any other suggestion? Thank you.
