Indeed, the mesh generated by the 3D Delaunay leads to elements that are
completely flat, which the optimizers cannot recover... We will keep
your file in our bug database.
Two solutions until we fix the problem:
1) Use the Frontal 3D algo, which works fine on your example
2) Extrude your mesh (cf. the "Layer" command in Extrude)
