[Gmsh] more tets than hexes in hex-dom mesh of cube

I attach another image showing the distribution of nodes within the
cube, hexDomCube_2.png. The green dots show the regular pattern of
nodes on the near face, the reds dots show the interior nodes.
Comparing this image with Fig 13(b) from the AMSES paper
http://www.amses-journal.com/content/1/1/8
it is clear that the distribution of the interior nodes in the cube is
considerably more irregular. Beyond the first layer the pattern looks
more like a Delaunay mesh. Does this indicate a problem with the vertex
insertion part of the algorithm?
