[Gmsh] Higher Order Extension

Hello,

I have a mesh generated in another program and a CAD model defining the
geometry.  I was hoping to use the higher order capabilities of Gmsh to
extend the original mesh to higher order.  I am able to merge the CAD model
on top of the mesh, but I don't know how to link the mesh to the CAD model
so that the extension to higher order will use the CAD model.  Is this
possible?  If so, how could I do it?
