[Gmsh] Inerescting Elements

Hi,

does anybody know what will cause the following warning:
WARNING: Intersecting elements {element number} and {element number}

its a 3D unstructured mesh
Mesh.Smoothing =3
Mesh.SmoothNormals = 1
Mesh.Optimise=1

the erorr  occurs before the optimisation steps.

any insight as to what it could be is very much appreciated.
